LIABILITY RELEASE WAIVER FOR ONLINE CLASSES

By filling out the form below and/or clicking on the box stating “I have read and agree to Donna's liability release form, terms, and conditions” when registering for class, you make the following agreements:

I understand that physical exercise, including flexibility training, pole dance fitness, yoga, sculpt, and inversion practices, can be strenuous and subject to risk of serious injury. I understand that I am urged to obtain a physical examination from a doctor before participating in any exercise activity.

I recognize that physical activity and exertion may be difficult and strenuous and may cause or aggravate a physical injury or medical condition.

I agree that I engage in any physical exercise or activity, I do so entirely at my own risk.

I acknowledge it is my responsibility to share previous injuries with your instructor, Donna Carnow, before class and/or at the beginning of class, before we get started. I understand that I am the resident expert on my body and am responsible for taking appropriate modifications as needed, taking breaks as needed, and/or asking for more information about more accessible options if the options offered to me in class do not feel safe or accessible now.

I acknowledge that this waiver and release of liability includes, without limitation, all injuries which may occur as a result of:

(a) My participation in any online activity or online training session and

(b) My instruction, training, and supervision by my instructor, Donna Carnow.

I acknowledge that I have carefully read this "waiver and release" and fully understand that it is a release of liability. I expressly agree to release and discharge Donna Carnow, LLC from any and all claims based on any injury or other harm I suffer in her class or causes of action. I agree to voluntarily give up or waive any right I may otherwise have to bring a legal action against Donna Carnow, LLC for personal injury or property damage.

I release Donna Carnow, LLC even if she was negligent.  If any portion of this waiver and release is deemed invalid for any reason, I agree that the rest of it is still valid. 

By agreeing to this waiver release, I acknowledge that I understand its content and that this release cannot be modified orally.
